SPORTS PHYSICALS

ALL STUDENTS participating in, or trying out, for any school related sports are required to have an up to date sports physical each year. 

Students can receive the examination with their family doctor OR

they can receive the examination with the school-based clinic. 

Students can stop by the clinic for a sports physical packet OR

print the packet yourself by clicking on the PDF below. 

Please fill out the packet and return as soon as possible, as appointments fill up quickly.
